Nominations invited from astronomy, cosmology, mathematics, and philosophy of science.; Individuals active in or appreciating contemporary cosmological research.; Self-nominations are not accepted.; Nominees must have made fundamental advances in understanding the universe.

Completed nomination form; Current curriculum vitae (WITHOUT full publication list); Letter of support from an individual outside the nominee's institution/research group
