Briefly describe dissertation.; Date of successful dissertation defense.; Date of degree award.; Name and contact information of primary dissertation supervisor.; Copy of dissertation as PDF or via link.; Published papers, papers in press, or portions thereof, based on dissertation research may accompany application.
