Demonstrate genuine interest in, and commitment to the blind of Illinois; Potential to develop such interest and commitment; Expected to attend all sessions relative to the event for which they receive assistance

Completely fill out the attached application; Submit in an accessible format (.doc, .docx, .txt, .rtf, or accessible Adobe PDF); Applicant must sign; Parent or guardian must sign a release if applicant is under 18
