Individual or group with significant impact on national health/mental health policy, professional standards, or exemplary program models.; Contribution may be in development, interpretation, implementation, institutionalization, or expansion of health or mental health policy or legislation.; Achievement should reflect positive improvements in social work profession and services to clients.; Award includes but is not limited to social workers.
Requirements
Completed nomination form.; Description of nominee's achievement related to health/mental health policy, its extent, and national impact.; Professional or programmatic attributes of the nominee.; How policy innovation improved service delivery and impact on individuals, families, groups, communities.; Evidence in legislation, regulations, research, teaching, administration, clinical standards, or public health/community mental health programs.; Three additional endorsements with support letters.
