Personal information (Social Security number, date of birth); Military history of the service member or Veteran sponsoring the application; Name of the preferred VA national cemetery for burial

Application materials

VA Form 40-10007 (Application for Pre-Need Determination of Eligibility for Burial in a VA National Cemetery); Copy of DD214 or other separation files (optional); Supporting files or affidavit for applying on behalf of someone else; Supporting files with information about an adult child's disability (if applicable)
