Application materials
Completed application form; Typed questions (extracurricular activities from the last two years with dates, significant awards or honors received during the last two years); Typed essay or video (750 words or 5 minutes maximum) telling your story, how your medical condition has affected your life, and explaining your goals for the future; Physician's statement of diagnosis (note stating diagnosis); Most recent official transcript (including the most current semester); Document verifying acceptance by college or current enrollment (acceptance letter or current enrollment documentation)
